Varicella Zoster Meningitis with Multiple Cranial Neuropathies: A Case Report and Literature Review

Abstract
Varicella zoster virus (VZV) infection is characterized by skin vesicles and neuralgic pain due to a trigeminal neuropathy. It may be complicated with aseptic meningitis and other cranial neuropathies. We report a case of multiple cranial neuropathies caused by VZV. A 73-year-old man presented with a temporal headache without skin lesion or fever. A few days after, he revisited the hospital due to persistent headache, skin eruptions, and facial palsy. Cerebrospinal fluid examination showed pleocytosis and positive VZV markers. The symptoms progressed to multiple cranial neuropathies involving III, IV, V, VI, VII, IX, and X. He received antiviral and steroid therapies and the symptoms had partially improved after a month. VZV infection may present with a wide variety of neurological symptoms, hence assessments for cranial neuropathies and early diagnostic planning for VZV meningitis are clinically important to reduce neurologic complications.
